NWHFSC Programs

The NWHFSC provides funding for family stability programs that provide free or low cost services for families. Our programs are focused on developing a continuum of coordinated family support services using a public health approach. By expanding programs and creating systems change,  we aim to increased positive health outcomes for children and families in the NWHFSC service  area.

Adverse Childhood Experiences (ACE's)
NWHFSC supports ACE education, training and presentations across the northwest region through partnerships with the MN Department of Human Services, Minnesota Communities Caring for Children, local partners, agencies, and providers.


Health Resource Center Coordinator
The Health Resource Center Coordinator is responsible for supporting the Brooklyn Center Health Resource Center through collaboration, basic needs, wrap around services, referral information, and data tracking.


NW Immunization Clinic
The NW Immunization Clinic program provides free immunizations to families and children who qualify for the MNVFC vaccine: baby shots and school vaccinations. The Immunization Clinic nurse also provides medical resources and referral information.


Stable Housing-Family Success Grant
Keeping your housing is critical to staying safe during the COVID-19 pandemic. If you live in Hennepin County and have lost income due to COVID-19, help may be available to cover current and past due housing costs such as rent, mortgage payments, and utilities.


Family School Coordinators
The Family School Coordinator program work is a regional design model in the six middle schools in the NWHFSC service delivery area. Family School Coordinators provide school-based mental health intervention services.


Lunch N Learn

Lunch N Learn meetings provide an opportunity to spotlight local organizations and programs. Participants in the meetings include social workers, clinic staff & family advocates in Hennepin County.


Regional Family Advocate
The Family Advocate works directly with families to provide support and resources. They provide referrals to Hennepin County and local community services for general and emergency needs including housing, food, clothing, and school information.
